John E. Ingram, MD

JOHN E. INGRAM, MD has been board certified in urology by the American Board of Urology since 1978. He graduated from the University of California at Los Angeles in 1972. He is a member of the California Medical Association, the Los Angeles County Medical Association and the Orange County Urological Association. He is a past Chief of Urology and also a past Chief of Staff at Long Beach Memorial Hospital. Presently, Dr. Ingram is a director of Memorial Health Services. He has been a member of MPT’s Quality Control Board since 1982 and currently serves as chair of that Board and as chair of the CAP Board of Directors. Dr. Ingram lives in Long Beach.

DANIEL BORENSTEIN, MD
DANIEL BORENSTEIN, MD is board certified by the American Board of Psychiatry and Neurology, is a Clinical Professor in the UCLA Department of Psychiatry and Biobehavioral Sciences and maintains a private practice. He is a Founder and past Director of the Foundation for the Advancement of Psychiatric Education and Research, and Founder and past Director of the UCLA Mental Health Program for Physicians in Training. Dr. Borenstein is past President of the American Psychiatric Association, the Southern California Psychiatric Society and the Bay District of the Los Angeles County Medical Association. He chairs the CAP Membership Development Committee and serves on the Board of the CAP Insurance Company.

PAUL GOTTLIEB, MD
PAUL GOTTLIEB, MD is a board certified urologist and has been in private practice since 1975. He graduated from the University of Witwatersrand in Johannesburg, South Africa and is a Fellow of both the Royal College of Surgeons in Edinburgh, Scotland and the American College of Surgeons. He is a member of the American Urological Society and the California Medical Association. He was chair of the Department of Urology at Mercy San Juan Medical Center and is currently Urological Advisor to the Northern California Chapter of the Multiple Sclerosis Society, Urological Consultant to the Medical Board of California, and Urology Consultant to the Mercy tumor board. He is a member of the selection board for candidates for the American College of Surgeons. Dr. Gottlieb resides in Carmichael.

VINCENT GUALTIERI, MD
VINCENT GUALTIERI, MD has been board certified in urological surgery since 1972. He graduated from the California College of Medicine in 1959 and obtained his residency training in urology at the Los Angeles County General Hospital. He is a past president of the Los Angeles County Medical Association (LACMA), a former trustee of the California Medical Association and is currently a member of the Boards of LACMA and the Los Angeles Foundation for Medical Care. In addition, he is a LACMA representative at the American Medical Association House of Delegates. He has also served as chair of MPT's Los Angeles Claims Review Committee since 1983. Dr. Gualtieri lives in Encino.

BÉLA KENESSEY, MD
BÉLA KENESSEY, MD is a board certified Diplomat of the American Board of Family Medicine, and a Fellow of the American Academy of Family Practice (FAAFP). Following graduation from the University of Southern California School of Medicine in 1989, he attended the USC/PIH Family Practice Residency Program where he served as Chief Resident from 1991-1992. There he was honored with the Resident Teacher Award by the Society of Teachers of Family Medicine. He currently remains a Clinical Instructor with the Dept. of Family Medicine at Keck School of Medicine, USC. He has a Bachelor of Arts in Economics/Business from UCLA and has served on several boards including his local hospital, as well as CFO for a community nonprofit organization and a Sleep Diagnostics Center. He has served as chair of MPT's Northern California Claims Review Committee since 2005. Dr. Kenessey established his family practice in San Ramon in 1992 and resides in Danville.

CARL MAGUIRE, MD
CARL MAGUIRE, MD received a B.S. and M.S. in Electrical Engineering from Purdue University. He subsequently worked in bio engineering prior to completing his medical training at the University of Missouri. He completed his orthopedic residency at the University of Wisconsin. He has been involved in the private practice of orthopedic surgery since 1976, with the exception of a period of spine fellowship at Twin Cities Scoliosis Center. At present, he practices in San Diego and is past Chief of Orthopedic Surgery at Sharp Memorial Hospital. In addition, he is a member of the American Academy of Orthopedic Surgery, the North American Spine Society, the Western Orthopedics Association, and the California Orthopedics Association. He obtained his board certification from the American Board of Orthopedic Surgery in 1977. Dr. Maguire was a member of the Orthopedic Surgical Practices Review Committee for four years and is currently a member of MPT’s Finance and Investment/Audit Committee.

MALCOLM PAUL, MD
MALCOLM PAUL, MD graduated from the University of Maryland School of Medicine in 1969. He completed his residency training in plastic surgery at George Washington University Medical Center in 1975, and was board certified in 1976. Dr. Paul is a Fellow of the American College of Surgeons and a Clinical Professor of Surgery at the Aesthetic & Plastic Surgery Institute at the University of California, Irvine and is Co-director of the Resident Aesthetic Surgery Training. He is the North American Editor for the Aesthetic Plastic Surgery Journal. Dr. Paul is past President of the Orange County Society of Plastic Surgeons and past President of the American Society for Aesthetic Plastic Surgery. He is Assistant National Secretaries Chair and U.S. National Secretary for the International Society of Aesthetic Plastic Surgery. Dr. Paul was also a Traveling Professor for the American Society for Aesthetic Plastic Surgery from 1998-2000 and 2002-2005. Dr. Paul is past President of the California Society of Plastic Surgeons. He has served as chair of CAP’s Education/CME Committee since 1997. Dr. Paul lives in Newport Beach.

GRAHAM PURCELL, MD
GRAHAM PURCELL, MD is a graduate of the University of Alberta, Canada. He completed his orthopaedic residency at UCLA in 1980 and the John Moe Spine Fellowship at the University of Minnesota in 1981. He has been in private practice in Santa Monica since 1984 and is currently an Assistant Clinical Professor of Orthopaedic Surgery at UCLA. Dr. Purcell is board certified by the American Board of Orthopaedic Surgery (1983). He is a Fellow of the American Academy of Orthopaedic Surgeons, North American Spine Society, and Scoliosis Research Society and is a member of the California Orthopaedic Association. Dr. Purcell is chair of the MPT Orthopaedic Claims Review Committee and is a member of the MPT Quality Control Board. He was an active participant in the CAP Fellows Program since its inception in 2003 and was elected to the CAP Board of Directors in June 2005. Dr. Purcell and his wife, Lezlie, reside in Studio City.

LAURIE REYNARD, MD
LAURIE REYNARD, MD is a board certified obstetrician/gynecologist in practice in Santa Monica since 1982. Dr. Reynard earned her undergraduate degree at Stanford University and graduated from USC School of Medicine in 1978. She obtained her residency training at Harbor-UCLA in Torrance and now teaches residents there as an Assistant Clinical Professor of UCLA. Active in the medical community, Dr. Reynard is on the voluntary staff of Venice Clinic, is an expert reviewer for the Medical Board of California, and serves on the LACMA District 5 Board. In addition to serving as a CAP Fellow for two years, Dr. Reynard is a member of the California Medical Association, Los Angeles OB/GYN Society, and serves on peer review committees at St. John's Hospital and Health Center in Santa Monica. Dr. Reynard currently lives in Los Angeles with her ophthalmologist husband and CAP member, Michael Reynard, MD.

STEWART SHANFIELD, MD
STEWART SHANFIELD, MD has been a board certified orthopedic surgeon since 1988 recertifying on at least two occasions. He attended medical school at the University of Texas Health Science Center at San Antonio, and completed his internship and residency at Los Angeles County-USC Medical Center. Dr. Shanfield has served in several elected and appointed positions at St. Jude Medical Center, including Chairman of the Board of Trustees, Chief of Staff, Chairman of Surgery, and an Assistant Clinical Professor of Orthopaedics at UCI Medical Center. In addition to his professional commitments, Dr. Shanfield is active in his local community serving as team physician for Sunny Hills High School, California State University Women’s Gymnastics Program, and a board member of Fullerton Community Bank. He serves as a volunteer medical director as a non sworn reserve police officer with the North Orange County SWAT team. In 2000, 2004, 2005 and 2006 the Orange County Medical Association honored him as a Physician of Excellence in orthopedics. His interests include arthroscopic surgery, joint replacement and restoration, sports related injuries and foot and ankle problems. He is in practice with Fullerton Orthopaedic Surgery Medical Group, Inc.

DAVID SIEVERS, MD
DAVID SIEVERS, MD practices surgical oncology with a sub-specialty in general surgery. He is board certified in surgery and graduated from the University of Southern California. Dr. Sievers is a Fellow of the American College of Surgeons, a member of the Los Angeles Surgical Society and a member of the Society of Graduate Surgeons. He is a clinical associate professor of surgery at USC School of Medicine. He serves as vice chair of the CAP Board of Directors and vice chair of the State Political Action Committee. Dr. Sievers lives in Encino.

J. HARTLEY TURPIN, MD
J. HARTLEY TURPIN, MD, is a graduate from the University of British Columbia in Medicine in 1956. Upon graduation and training, Dr. Turpin served as a flight surgeon in the Royal Canadian Air Force in France from 1959 to 1962 and immigrated to the United States in 1965, becoming a citizen in1984. Dr. Turpin was a partner in the Southern California Permanente Medical Group until 1978 when he went into private practice and joined CAP. He is in solo family practice in Newport Beach. Currently, he serves on MPT's Quality Control Board, and chairs the Primary Care Peer Review Committee. He also is the chair of CAP’s State Political Action Committee, represents CAP on the board of Californians Allied for Patient Protection in Sacramento, and serves on that organization’s Political Action Committee. Among his many hobbies and interests, Dr. Turpin is a life-long sailor and still participates in a number of annual tournaments and competitions.

PAUL "ROCKY" WEBER, MD
PAUL "ROCKY" WEBER, MD is a board certified obstetrician/gynecologist who has practiced at Women’s Hospital/Long Beach Memorial Medical Center in Long Beach since 1981. He received his medical degree in 1977 and his PhD in 1974 from Jefferson Medical College in Philadelphia, PA. In 1981 he completed his residency training in obstetrics/gynecology at Harbor-UCLA Medical Center in Torrance, CA. He is an Associate Clinical Professor at University of California, Irvine, School of Medicine. While at Women’s Hospital, he has held positions on the Board of Trustees, Board of Directors, Chief of Staff, Chief of Obstetrics and has been a long time member of the Executive Committee. He is presently Chief of Staff at Women’s Hospital. He is currently in a one-year fellowship program for physician leaders sponsored by Memorial Health system. As a CAP member for 25 years, he has served on the Obstetrical Claims Review, Obstetrical Peer Review, Quality Control Board, and Education Committees. Dr. Weber and his wife, Barbara, live in Rancho Palos Verdes, and they have three children, Matt, Nick and Paige who are at school in San Diego, Santa Barbara and Berkeley respectively.

BRUCE WEIMER, MD
BRUCE WEIMER, MD is a board certified neurologist in private practice in Glendora. Dr. Weimer received his pre-med undergraduate, medical graduate and neurology training at the University of Pittsburgh in Pittsburgh, Pennsylvania. After serving as chief resident in Neurology, he joined the University of Pittsburgh Neurology Department as a Clinical Instructor and researcher. In 1985, Dr. Weimer moved to Southern California to establish his private practice. He is a Diplomat of the American Board of Neurology and the National Board of Medical Examiners. Dr. Weimer currently serves on CAP’s Claims Review Committee, Education Committee and Federal Political Action Committee. A member of Foothill Presbyterian Hospital's Medical Executive Committee, Dr. Weimer also serves as chairman of their Physician Well-being Committee and was recently recruited to serve on the hospital’s Palliative Care Committee. Dr. Weimer currently resides in West Covina.

TAMMY WU, MD
TAMMY WU, MD is a board certified Modesto plastic and reconstructive surgeon who, along with husband and practice partner, Dr. Calvin Lee, is an active member of CAP’s Central California District Council. Dr. Wu graduated as the top female medical student from the Program in Liberal Medical Education (PLME) at Brown University, a curriculum that encourages medical students to pursue their interests in the humanities, social sciences, and natural sciences as they prepare for their careers as physicians. Additionally, she is a graduate of the Southern Illinois University Plastic Surgery Training Program. Currently on staff at UC Davis as a Volunteer Assistant Professor, Dr. Wu also teaches several rotating medical students, physician assistant students, and nursing students. A Modesto resident, Dr. Wu runs a program called Surgical Artistry Academy which teaches the community about medicine in general and cosmetic surgery.
