Reportable Events Table

The following examples represent the type of events you should report
to CAP:
  • Birth related events with symptoms of birth related injuries.
  • Unexpected death of a patient during hospitilization, treatment, or in close proximity to the time of their last office visit.
  • Unexpected, severe complications arising during and/or after patient care or treatment, i.e.,
    a. Brain damage.
    b. Quadriplegia.
    c. Paraplegia.
    d. Blindness
    e. Hemiparesis.
    f.  Non-preexisting conditions that negatively impact the patient's     lifestyle, prognosis, or longevity.

  • Surgical misadventures, such as:
    a. Wrong site or side of the body
    b. Retained foreign bodies, i.e., sponges, needles, or surgical tools.

  • Blood product related events (not including allergic reactions)
    a. Type and cross mismatch
    b. Transmission of blood born pathogens, i.e., HIV or hepatitis

  • In-office patient injuries
