CAP PRIVILEGES ONLINE

The Cooperative of American Physicians, Inc. (CAP) provides hospitals and other third parties direct access to credentialing documents for our members through CAP Privileges Online. This program is just another way CAP is committed to enhancing the efficiency of handling administrative tasks associated with a medical practice.

As an enrolled participant in this program, you will have continuous electronic access to CAP member physician certificates of coverage and claims history documents for which a signed consent form has been provided.

To apply for enrollment in CAP Privileges Online, please complete the Account Access Information form and fax it to 213-473-8773. A Membership Services representative will contact you shortly thereafter to confirm receipt and continue the online enrollment process.
